Hammer losing head...

Just got back from a weekend trip and notice one of the heads on a new hammer literally fell off.

In pics, the head is on the sand and the head / skeleton has some brown and a bit melty on the other side.

I did a water change before leaving...and only thing diff is I dosed Kent essential nutrients.

Other euphyllias are fine... concern is if this might be a sign of a problem that will affect the others... I've have euphyllias melt on me before but never heads (soft tissue part) fall off before...

Any ideas?

could it be brown jelly disease? what a coincidence, I was just talking with someone who lost her gold hammer colony the same way yours is going.
